Terms and Conditions for Hollywood Smile Treatment
Bright Dental Care – Bossley Park

1. Treatment Coverage
   The quoted price for the Hollywood Smile procedure applies exclusively to cases requiring 10 teeth per jaw (either upper or lower).

• If fewer than 10 teeth are being treated in a single jaw, the standard Hollywood Smile pricing does not apply, and a custom quote will be provided based on individual requirements.

2. Treatment Scope
   The Hollywood Smile package typically includes:

• Consultation and for the Hollywood requirements.
• Tooth preparation session and construction of Temporary teeth cover after teeth’s colour selection.
• Crowns for 10 teeth per jaw,. 
• Final fit and adjustment

Note: Any additional treatments such as fillings, root canals, or gum treatment are not included in the price and will incur separate charges.

3. Warranty
   A 2-year limited warranty is provided for the Hollywood Smile treatment, covering:

• Manufacturing defects
• Fractures or de-bonding of crowns under normal use

Warranty Conditions:

• Patients must attend regular 6-monthly check-ups at Bright Dental Care.
• Warranty is void if damage is caused by neglect, trauma, or misuse (e.g. biting hard objects, grinding without a night guard).
• Smoking or lack of proper oral hygiene may void the warranty.

4. Payment Terms
   Bright Dental Care accepts the following payment methods:

• EFTPOS / Credit Card
• Cash
• Settlement payments (e.g. split the payment into 2-3 times)

5. Cancellation & Refunds

• A deposit may be required to secure treatment appointments.
• Deposits are non-refundable if cancelled within 48 hours of appointment time.
• Once treatment begins, refunds are not available for completed stages.

6. Consent & Acknowledgement
   By proceeding with treatment, patients acknowledge they have received a full explanation of the procedure, risks, alternatives, and costs. Informed consent must be signed before treatment begins.
